AIMS AND GOALS

 

 

1.                  To expand and  maintain a regular Plastic & Reconstructive Surgical Service to those patients with physical disability in the West Timor region of Nusa Tenggara Timur (NTT), Indonesia

 

 

2.                  To work with the local counterpart medical and nursing staff at the Rumah Sakit Katolik Marianum to maximise the teaching and skills transfer of the management of patients with physical disability and deformity.

 

3.         To implement the training programme in cleft lip and palate surgery for a Timorese surgeon. 

 

 


 

INTRODUCTION

 

Beginning in 2007, the RSKM hospital complex in Halilulik, West Timor became the second site at which OSSAA teams deliver a plastic and reconstructive surgical service to the people of Nusa Tengarra Timur (NTT).  In an effort to spread this service equitably across the province and maximise delivery of this surgery to those most in need, it is hoped to expand it further by activating a third location in Lela, Eastern Flores later in 2009.

 

This visit the 4th plastic surgical team here since 2007 continued the focus on the management of cleft lip and palate in the region.  Extending the mission of the team from simply clinical service delivery, we on this visit commenced a programme teaching a Timorese surgeon cleft lip surgery.

 

Whilst the focus of this visit was principally in Halilulik, a pilot orthopaedic surgical visit was undertaken at nearby Naob, overseen by our Indonesian counterpart Dr Harianto.

This visit was coordinated by the SSpS sisters who administer this hospital facility, at the invitation of the local government authorities.  Assistance with the team travelling through Timor Leste to West Timor was provided by the resident RACS staff in Dili  

 

 

                                                      


 

OVERVIEW

 

With the two-fold purpose of providing a clinical service and the commencement of a teaching programme in cleft lip and palate surgery for a Timorese surgeon, this team approached Halilulik via Timor Leste.  Departing Darwin on an Air North flight, the team arrived in Dili where it was met by Dr Eric Vreede (Leader ATLASS project) and Dr Joao Ximenes (Timor Leste surgeon).  Transport and a driver had been previously arranged allowing the team to travel directly on to Batugade and crossing the border into Indonesia.  The visas organised from the Darwin Indonesian Consulate allowed uncomplicated passage across the border.  Indeed, an Indonesian customs official recognised members of our team, as his son with a burn contracture had been previously treated by OSSAA teams in Cancar, Flores.

 

The SSpS sisters met the team and facilitated our onward transfer to Halilulik where we arrived mid afternoon.

 

Consultation with a number of assembled patients occurred soon after arrival.  During these assessment clinics the team was accompanied by the young resident Indonesian doctors – this provided a welcome additional teaching opportunity.

 

Whilst the initial turnout of cases on Saturday and Sunday was smaller than anticipated, as the news of the team’s presence was further disseminated, more patients presented and a full week’s operating planned.  It appears that either there are fewer cleft patients than expected in this region, or more likely that there are unresolved difficulties in distributing details of the team’s visit to the more remote locations, especially when compared to Cancar.

 

The operating theatre, newly commissioned before the December 2008 visit was again in use and all supplies and equipment left from that visit were available to the team.  The new Penlon anaesthetic machine was employed all week without event – the team’s anaesthetist using the OMV portable machine for several cases on the final day as a teaching and learning exercise.  Local nursing staff and sisters assisted in all cases and the resident junior doctors and some colleagues from Atambua and the surrounding districts also frequently attended theatre.

 

The post operative care of all patients occurred uneventfully in the wards of RSKM, supervised by Sr Josepha and St Helma.  All cleft patients were discharged by the second post operative day and only one burn contracture release case would require ongoing care by local staff after the team’s departure.

 

The clinical focus, as with previous visits, was on the surgical treatment of cleft lip and palate patients.  Whilst most cleft lip cases were in the younger age bracket (less than 1 year), there were still three adult unrepaired cleft cases who presented for treatment.   This suggests that there remains a large pool of untreated clefts still in this region.  All cleft lip patients had their anaesthesia delivered via a laryngeal mask, this now becoming the most common approach of our teams.  This has to date proved safe intra-operatively and assists with recovery and early post operative care, while dramatically improving surgical turnover time.

 

The cleft teaching programme for Dr Joao Ximenes commenced with a focus on incomplete unilateral cleft lip.  By week’s end he had successfully performed six such unilateral cleft lip repairs in concert with the senior surgeon – this possibly represents the first cleft repairs performed by a Timorese surgeon.  It is hoped to progress Dr Joao’s training later this year, whilst in the interim he commence these simple cases by himself on his return to Dili.

 

The number of cleft palate patients continues to grow, with many of these being patients happy with their lip repairs on a previous visit.  Two patients with isolated cleft palate, identified because of their major speech deficits, also presented for treatment.  One of these was an adult, a trainee SSpS sister based in Halilulik.

 

The only burn case requiring treatment was the teenager whose unhealed chest wall burn wound was skin grafted on our visit in December 2008.  Whilst the graft was well healed, his severe anterior neck contracture was in need of wide release and grafting.  Post operatively he was splinted in extension, and detailed description for aftercare left with the local nursing staff.

 

Several cases of perioral and nasal soft tissue deformity following childhood infection (noma, cancrum oris) were assessed.  The first stage of their reconstruction was commenced with local soft tissue flaps.  These cases will likely require repeat and revisionary surgery to achieve the desired final outcome and will be re-assessed on future visits.

 

Drs Harianto, Keenan and the Naob team visited briefly on the Wednesday, the same day that our team was privileged to a 3 hour church service celebrating 12 SSpS sisters taking their final vows.

 

Dr Harianto returned on Friday to perform a list of local anaesthetic excisions of small skin lesions and lipomas.  He undertook these with the assistance of the local Indonesian junior doctors.

 

All surgical procedures during the week proceeded without event.  One young child had her surgery deferred for several days when it became apparent at induction of anaesthesia that she had not been properly fasted – the dangers of this were communicated to the family and staff.

 

At week’s end the team discussed with Sr Helma and her colleagues the types and frequency of surgical service needed at RSKM – in summary there was agreement that a general surgical team focussing on hernias and other similar level problems would be welcome, especially as there is no resident general surgeon in Atambua at present.  Plastic surgical team visits will continue on a once a year basis.

 

The team returned via the same route taken to enter West Timor, assisted by the SSpS sisters and the vehicle and driver from Dili.  An overnight stay in Dili allowed a short discussion with Dr Eric Vreede (RACS team leader) about Dr Joao Ximenes cleft surgical training and the expectations of him over the next few months.

 

Return to Australia occurred on Sunday 28 June, allowing Dr Moore and Ms Boveington to meet with the Indonesian Consul and his staff in Darwin, to update them on OSSAA activities in NTT.

 

 

SUMMARY OF THE VISIT

·         The initial steps in a programme of teaching cleft lip and palate surgery to a Timorese surgeon.

·         More efficient travel to and from Halilulik via Timor Leste

 

·         The continued support from the local and provincial governments for the OSSAA team work in Halilulik.

 

·         The continued support of the Indonesian Consul in Darwin for the provision of appropriate visas and documentation to allow easy travel within Indonesia.
